Emerging Connection Found Between Dementia and Acid Reflux Medications

Previously associated with a heightened susceptibility to stroke and heart attack, popular acid reflux medications have now been linked to an increased potential for developing age-related dementia.

These medications fall under the category of proton pump inhibitors (PPIs), designed to target proton pumps that produce stomach acid. By doing so, they prevent excessive acid flow into the esophagus, averting heartburn and other severe gastrointestinal complications that could lead to conditions like cancer.

PPIs are among the most commonly used drugs worldwide, with an estimated 15 million Americans consuming them annually. These medications are available both through prescription and over-the-counter.

In the United States, they include Omeprazole (Prilosec, Prilosec OTC, Zegerid), Esomeprazole (Nexium, Nexium 24HR), Lansoprazole (Prevacid, Prevacid 24HR), Dexlansoprazole (Dexilant), Pantoprazole (Protonix), Rabeprazole (AcipHex), and Esomeprazole/Naproxen (Vimovo).

"Proton pump inhibitors are a valuable tool to manage acid reflux. However, long-term usage has been previously linked to a higher risk of stroke, bone fractures, and chronic kidney disease," explained study author Kamakshi Lakshminarayan, from the University of Minnesota School of Public Health in Minneapolis. "Although no link was found with short-term usage, we did observe a higher dementia risk with extended PPI use."

Researchers from the American Academy of Neurology studied 5,712 participants aged 45 to 64 in the Atherosclerosis Risk in Communities (ARIC) Study. These individuals were dementia-free at their initial health assessment between 1987 and 1989.

Over a quarter of the participants, approximately 1,500 individuals, took PPIs during the study period. After accounting for variables like age, sex, race, and comorbidities such as high blood pressure, the researchers determined that those using PPIs for over 4.4 years had a 33% greater risk of developing dementia compared to non-users, out of 497 participants reporting prolonged PPI use (more than four years and four months), 58 developed dementia.

However, there was no increased risk for those with intermittent PPI use or usage lasting less than the critical threshold of four years and four months.

Importantly, this study does not establish causation but instead prompts further investigation into long-term medication usage and potential mechanisms prompted by proton pump inhibition.

"While alternative treatments like antacids, weight management, and dietary changes are available for acid reflux, it's important for individuals taking these medications to consult their healthcare provider before making any changes. Abruptly discontinuing these drugs could exacerbate symptoms," advised Lakshminarayan. H2 blockers are also alternatives to PPIs; they bind to histamine type 2 receptors on the surface of gastric parietal cells, disrupting acid secretion and production.

Nevertheless, the researchers cautioned against hasty medication alterations, pointing out study limitations including potential inaccuracies in self-reporting and confounding factors like depleted B12 levels (which were not evaluated in participants). Notably, not all prior research on PPIs and cognition has yielded definitive adverse outcomes.

"Further research is necessary to validate our findings and explore the potential connection between long-term proton pump inhibitor use and a heightened dementia risk," stated Lakshminarayan.

The findings were published in the journal Neurology.
